Organisation: Tsohang Youth Project

Tsohang Youth Project is a community-based Non-Profit Organization focused on youth empowerment, substance use disorder awareness, public health, human rights, and advocacy for people who use/inject drugs. We promote and defend the rights to equality, dignity, and freedom regardless of race, gender, education, health, or economic status. Our work includes linking communities to treatment centres, therapy, harm reduction centres, job readiness workshops, local teams, organized groups, arts groups, indigenous games, and more.

Role of the Community Monitor

The Community Monitor plays a critical role in gathering real-time data and feedback from health facilities and communities to ensure accountability, improve service delivery, and strengthen HIV, TB, and related health programmes. This position supports evidence-based advocacy by documenting barriers, gaps, and successes in accessing health services.

Key Responsibilities

1. Data Collection and Monitoring

  • Collect consistent, robust qualitative and quantitative data at site level.
  • Conduct regular visits to health facilities and community sites to monitor service delivery.
  • Lead data collection processes such as interviews and observations.

2. Stakeholder Engagement
  • Engage community members, key populations, and civil society groups to gather feedback.
  • Engage clinic staff, clinic committees, healthcare users, and other stakeholders to advocate for improved service delivery.
  • Support the organisation in participating in accountability meetings and structures.
3. Reports and Documentation
  • Compile and submit accurate monitoring reports within agreed timelines.
  • Escalate urgent issues (e.g., medication stock-outs, rights violations) to the Project Coordinator.
4. Support Advocacy Efforts
  • Share findings with community structures and health facility managers.
  • Participate in district-level review meetings to present evidence and advocate for improvements.
5. Capacity Building
  • Attend training on CLM tools, data quality, and human rights.
  • Support peer learning among community monitors and stakeholders.
